You simply fill in the required information below and submit the form to us. We will then contact you to make an appointment for the prearrangement agreement and pet cremation authorization. Payment can be made by credit card, cash, or check. Your payment will be placed in a bank account for future use. When the time comes to use our services, we will be paid from the bank account. If for any reason, you should decide to cancel the prearrangement after three days from the date of the agreement, you will receive a refund of 30% of the purchase price.
